I, as the legal Parent, Guardian of the child mentioned above, do hereby release, the DC Way Academy, its employees, agents, officers, and managers from any and all claims arising from personal injury no matter how caused, which my child may incur or suffer during my child's participation in DC Way Academy soccer programs and DC Way Academy. I hereby waive any claims, suits, actions, or causes which I or my child may or hereafter have against Denis Chekuristov, DC Way Academy, its owners, employees, agents, officers, and managers for personal injury, no matter how caused which I or my child may have incurred, may incur or suffer during my child's participation in soccer drills, scrimmages or sessions. I further agree to indemnify and hold forever harmless, Denis Chekuristov, DC Way Academy, its owners, employees, agents, officers, managers, and successors against all losses, including counsel fees and court costs, from any and all claims made against it by any party as a result of my or my child's actions, negligent or intentional, which may result in the injury or loss to another participant or spectator. In addition, in the event that I cannot be reached, I hereby give permission to the Program Director to obtain appropriate medical treatment for my child. I, the parent (guardian) of the child mentioned above, give permission for the named player to receive emergency medical or surgical treatment and hospitalization if necessary. I understand that every attempt will be made to contact me, or the emergency contact named above, before taking this action. I will be financially responsible for any medical attention needed during a program or resulting from an injury received at the soccer program. My medical insurance shall be the insurance coverage for any medical treatment. I further agree that my child can receive over-the-counter remedies. (Tylenol, Sudafed, etc.) I HAVE READ THIS HEALTH FORM AND RELATED CERTIFICATIONS, THE RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND THEIR TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND SIGNING IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

COVID-19: the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. DC Way Academy has put in place preventative measures to reduce the spread of COVID-19. However, DC Way Academy cannot guarantee that I, my child(ren), or anyone else will not become infected with COVID-19. Participating in a DC Way Academy athletic program or activity could increase the risk of contracting COVID-19. In consideration of being allowed to participate in any way in any DC Way Academy activities, I understand, acknowledge, and agree to the following: 1. I knowingly and willingly consent to be coached by DC Way Academy coaches and staff and any designated associates and employees. 2. I understand the COVID-19 virus may not show symptoms. It is impossible to determine who has COVID-19 and who does not, given the current limitations. 3. Risk of transmission: I understand that due to the frequency of contact during the training and participation of soccer between coaches and players, characteristics of the virus, and characteristics of training with DC Way Academy, I have an elevated risk of contracting the virus by simply participating in this training, even though standard precautions are being observed. 4. I am unaware of being a possible carrier or infected: I confirm that I have not tested positive for COVID-19 in the last 30 days and that I am not presenting any of the COVID-19 symptoms. I. Fever of 100.5 degrees Fahrenheit or 37 degrees Celsius or higher II. Shortness of breath III. Dry cough IV. Runny nose V. Sore throat VI. A diminished sense of taste or smell 5. Contact with the infected: I confirm that I have not knowingly been in close contact with someone who has tested positive for COVID-19 in the last 14 days, or with anyone that has had the above-stated symptoms in paragraph 4 (#4) in the last 14 days. 6. I confirm that I have had no known exposure to the virus in the last 14 days. INFORMED CONSENT: I have been given the opportunity to ask any questions regarding the risks of contracting COVID-19 from DC Way Academy and participation in their practices and activities. I reaffirm that I am not a carrier of COVID-19 nor infected with COVID-19 to the best of my knowledge, including those in the household. I voluntarily assume any and all risks, which may be associated with any phase of my training as a result of the COVID-19 pandemic. I acknowledge the nature and purpose of the training procedures recommend under the current circumstances and restrictions that have been explained to me and that I have been given the opportunity to ask questions. ; By signing this agreement, I ACKNOWLEDGE the contagious nature of COVID-19 and VOLUNTARILY ASSUME THE RISK that I may be exposed to or infected by COVID-19 by participating in a DC Way Academy practice or activity, and that such exposure or infection may result in personal injury, illness, and more. I understand that the risk of becoming exposed to or infected by COVID-19 at a DC Way Academy practice or activity may result from the actions, omissions, or negligence of myself or others, including but not limited to DC Way Academy employees, volunteers, and program participants. I UNDERSTAND AND VOLUNTARILY ACCEPT AND ASSUME ALL the foregoing risks related to COVID-19 and accept sole responsibility for injury or illness that may occur. Further, I UNDERSTAND AND AGREE that this release includes any Claims based on the actions, omissions, or negligence of DC Way Academy, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any DC Way Academy practice or activity. I, the undersigned, accept.

I grant DC WAY Academy the right to take photographs and videos of me and my family in connection with the above-identified event. I authorize DC WAY Academy and its assigns and transferees to copyright, use, and publish the same in print and/or electronically. I agree that DC WAY Academy may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, and Web content. I have read and understood the above release.
